I have the rules under m'y eyes and its wroten : only the adjacents donut change thé face durin a insertion. During my last game, three donuts are changed when my adversary insert a donut.

I've checked the replay and everything is ok.
"Adjacents donuts" means adjacent to the insertion. The insertion is not composed of only one donuts, if you place a donut and close to another donut of yours, you can create an insertion of 2 donuts between opponent ones. So it means that the adjacent donuts of this insertion will be flipped.
State of the game : X_0X0
If 0 plays on _ : X(00)X0 the insertion is between parentheses.
So we have to return X , the result will be 00000 and 0 wins.
