I have the option "End of turn confirmation" on.
I upgrade my engine which forces me to take a debt.
There is no turn confirmation and no way to undo.

I actually wanted to do exactly that but that is beside the point; I have turn confirmation on to avoid missclicks and miscounts causing me to take a debt I don't want to.
As a side note, an option that would have turn confirmation on only if debts are taken could be welcome to some players.

If I remember correctly, it did ask me if I wanted to upgrade my loco but that is not an "end of turn confirmation" and it didn't warn me that it implied taking a debt.

The upgrade engine primary button, causes a secondary confirmation dialog to be displayed.
Clicking 'OK' is the end of your turn, and that's enough confirmation.
Debt is a valid game tactic, and can be incurred in a variety of ways.
Tactical decisions are the gamers choice.
